Who individual therapy fits
It is the right starting point for most adults with OCD. You may be a fit for individual therapy if:
- You have never tried therapy specifically for OCD, or you have tried general talk therapy and it has not changed the loop;
- You want one-on-one privacy and pacing, especially for content (intrusive harm thoughts, sexual orientation OCD, religious or moral scrupulosity) that feels too vulnerable to bring to a group right away;
- Your symptoms are interfering with work, school, or relationships, but you are still functioning;
- You have a co-occurring presentation — anxiety, depression, perfectionism, ADHD — that needs to be addressed alongside OCD.
What a session actually looks like
The first two or three sessions are a careful clinical assessment. Your clinician maps the obsessions you struggle with, the compulsions you use to neutralize them (including the ones that happen entirely in your head), the situations that trigger you, and the avoidances that have crept in. From there, we build a personalized exposure hierarchy and a written treatment plan.
Ongoing sessions follow a predictable structure: brief check-in, agenda-setting, a planned exposure (in-session or planned for the week), and homework. You will not be sprung on. Nothing happens without your buy-in. ERP works because it is collaborative, not because it is forced.
